Interpretation

Complement function via the classical pathway, is dependent on the presence of each component of the classical cascade, C1q through C9. While measured C3 and C4 concentrations may be normal, complement function may nevertheless be deficient owing to the absence of one particular component of the cascade.

CH50 is an adequate screen for congenital lack of a complement component. Genetic defects result in loss of or very low levels of a components activity, so that haemolytic activity approaches zero.

By contrast, in acquired deficiencies, CH50 may be normal or only slightly depressed in the face of significant loss of a particular component and therefore the test is not useful for serial monitoring.

Additional Information

Send frozen (out of town only) serum, separated ASAP after bleeding.

Requests for C2 & C5-9 will be tested for CH50 and the individual components will only be tested if CH50 is deficient and the Clinical Immunologist approves sending testing overseas. Additional samples may be required and costs and turnaround time will be increased if samples are sent away.

Alternative pathway and Lectin binding pathways will also be reported.
