Interpretation

 

For<2.5 fold elevations
 
Elevated plasma (nor)metanephrines – contributory factors may include stress of any acute illness, renal impairment and interfering medications (eg beta-blockers, tricyclic antidepressants). In the first instance, consider repeat test after fasting, avoidance of caffeine and nicotine, omission of interfering drugs or after lying supine for 30 minutes.
 
For >2.5 – 3 fold elevations
 
Elevated plasma (nor)metanephrines – contributory factors may include stress of any acute illness, renal impairment and interfering medications (eg beta-blockers, tricyclic antidepressants). In the first instance, consider repeat test after fasting, avoidance of caffeine and nicotine, omission of interfering drugs or after lying supine for 30 minutes.   Suggest endocrine review if suspicion of phaeochromocytoma remains.
 
For > 3 fold elevations
 
Markedly elevated (nor)metanephrines.   Suggest endocrine review.
Additional Information

Grossly lipemic and haemolytic samples should be avoided.

Test includes both plasma Metanephrines and Normetanephrins

 

Assay for plasma 3-methoxytyramine (3-MT), a dopamine metabolite associated with SDH mutations and more malignant potential, is also available upon request.
